India faces Twitter backlash over Internet clampdown

The backlash came as New Delhi turned up the heat on Twitter, threatening "appropriate and suitable action" if it failed to remove the accounts as soon as possible. Several Indian newspapers said this could mean a total ban on access to Twitter in India but government officials would not confirm to Reuters that such a drastic step was being considered.

There was no immediate response from Twitter, which does not have an office in India. There are about 16 million Twitter users in the South Asian country.

The government has found itself on the defensive this week over what critics see as a clumsy clampdown on social media websites - including Google, YouTube and Facebook - that has raised questions about freedom of information in the world's largest democracy.

"Dear GOI (Government of India), Keep your Hands Off My Internet. Else face protest" tweeted one user, @Old_Monk60.

India blocked access to more than 300 Web pages after threatening mobile phone text messages and doctored website images fuelled rumors that Muslims, a large minority in the predominantly Hindu country, were planning revenge attacks for violence in the northeastern state of Assam, where 80 people have been killed and 300,000 have been displaced since July.

Fearing for their lives, tens of thousands of migrants fled Mumbai, Bangalore and other cities last week. The exodus highlighted underlying tensions in a country with a history of ethnic and religious violence.

According to documents obtained by Reuters, the government has targeted Indian journalists, Britain's Daily Telegraph, the Australian Broadcasting Corporation and Al Jazeera television in its clampdown on Internet postings it says could inflame communal tensions.

The directives to Internet service providers listed dozens of YouTube, Facebook and Twitter pages. A random sampling of the YouTube postings revealed genuine news footage spliced together with fear-mongering propaganda.

INDIAN JOURNALISTS TARGETED

The government says Google and Facebook have largely cooperated while Twitter has been much slower to respond.

"Every company, whether it's an entertainment company, or a construction company, or a social media company, has to operate within the laws of the given country," said Sachin Pilot, minister of state in the Ministry of Communications.

Twitter has been instructed to remove 28 pages containing "objectionable content", an interior ministry official said.

"If they do not remove the pages, the Indian government will take appropriate and suitable action," he added.

The government has ordered Internet service providers to block the Twitter accounts of veteran journalist Kanchan Gupta and television anchor Shiv Aroor. Some appeared to have begun complying with the order on Thursday as Twitter users reported difficulties in accessing their pages.

"It is a political decision, because of my criticism of the government," said Gupta, who was an official in the previous government led by the Hindu nationalist Bharatiya Janata Party.

The government's actions triggered a storm of criticism from Twitter users, with the hashtags #Emergency2012 and #GOIBlocks among the top trending topics on Twitter in India on Thursday. Some compared the situation with the state of emergency imposed by the government in 1975, when some journalists were jailed.

The Centre for Internet and Society, which analyzed the 300 banning orders, found that they contained "numerous mistakes and inconsistencies". Some of the banned websites belonged to people trying to debunk the rumors, for example, it said.

"This isn't about political censorship. This is about the government not knowing how to do online regulation properly," said CIS program manager Pranesh Prakash.

India's parliament last year passed a law that obliges Internet companies to remove a range of objectionable content when requested to do so, a move criticized at the time by rights groups and social media companies.

Pay for performance may improve treatment implementation for adolescent substance use disorders

ScienceDaily (Aug. 13, 2012) ? Pay for performance appears to be associated with improved implementation of an adolescent substance use treatment program, although no significant differences were found in remission status between the pay-for-performance and implementation-as-usual groups, according to a report published Online First by Archives of Pediatrics & Adolescent Medicine, a JAMA Network publication.

Pay for performance (P4P, when financial incentives are given for achieving predefined criteria) is a strategy recommended by the Institute of Medicine to help improve the delivery of high-quality care. While the number of P4P programs in theU.S.has increased (one study suggests more than 150 such programs exist), the increase has occurred largely without randomized controlled studies to evaluate P4P approaches, according to the study background.

Bryan R. Garner, Ph.D., and colleagues of the Lighthouse Institute, Chestnut Health Systems, Normal, Ill., report the main effectiveness findings from a cluster randomized trial to evaluate the efficacy of P4P methods to improve treatment implementation and effectiveness.

In the study, 29 community-based treatment organizations were assigned to either the implementation-as-usual (IAU) control group or P4P. Each organization delivered the same behavioral treatment program, the Adolescent Community Reinforcement Approach (A-CRA), a program designed to reward nonsubstance-using behaviors so they can replace substance-use behaviors. Therapists in the P4P group were paid $50 for each month they demonstrated competence in treatment delivery (A-CRA competence) and $200 for each patient who received a specific number of A-CRA procedures within a certain time period (target A-CRA), according to the study.

Adjusted analysis results indicate that therapists assigned to P4P had a "significantly higher likelihood" of demonstrating A-CRA competence compared with therapists assigned to IAU (24 percent for P4P vs. 8.9 percent for IAU). Patients in P4P also had a "significantly higher likelihood" of receiving target A-CRA compared with patients assigned to IAU (17.3 percent for P4P vs. 2.5 percent for IAU). However, "no statistically significant difference" in patient remission status was seen between the two groups (41.8 percent for P4P vs. 50.8 percent for IAU), according to study results.

"Findings from this trial suggest that P4P can be an effective method of improving implementation of evidence-based treatment in practice settings. As hypothesized, we found that offering monetary bonuses directly to therapists had a large effect on increasing their demonstration of (1) monthly competency in implementing treatment procedures with patients and (2) the delivery of a predefined threshold level of treatment to adolescent patients," the authors note.

Because the effects of P4P intervention associated with treatment implementation did not translate to a significant difference in patient treatment effectiveness (i.e. remission status), researchers conducted post hoc analyses to evaluate the association between A-CRA competence and target A-CRA with remission. They suggest that therapist-level A-CRA competence was not significantly associated with patient remissions status, but that patient target A-CRA was "significantly associated" with remission status, according to the results.

"Pay for performance can be an effective method of improving treatment implementation," the study concludes.

Editorial: Disparities in Health Care

In an editorial, Alyna T. Chien, M.D., M.S., of Boston Children's Hospital and Harvard Medical School, Boston, writes: "Although much more work must be done to connect improved care processes with desired clinical outcomes, this study supports the notion that frontline providers respond to piece-rate P4P incentives related to improving care processes in the treatment of children."

"Where do the findings of Garner et al fit in the broader landscape of experiments with P4P? First, large gaps in our understanding of the effectiveness of P4P strategies persist even though the number of stakeholders and the circumstances in which P4P tactics are being used continue to proliferate rapidly," Chien continues.

"The most recent catalyst for research into outstanding questions about P4P is the rise of accountable care organizations in theUnited States. Accountable care organizations will seek to maximize their earnings in contracts that combine P4P incentives (to improve quality) with risk-based capitation (to reduce spending), and payers, providers and policy makers will all want better evidence about appropriate ways to target and design P4P incentives for a variety of common conditions to structure fair and clinically meaningful agreements," Chien concludes.

That formation, named Mount Sharp, stands at the center of the vast, ancient impact crater and several miles from where Curiosity touched down at the end of an eight-month voyage across 352 million mile (566 million km) of space.

The layers of exposed rock are thought to hold a wealth of Mars' geologic history, making it the main target of exploration for scientists who will use the rover to seek evidence of whether the planet most similar to Earth might now harbor or once have hosted key ingredients for microbial life.

But mission controllers at the?Jet Propulsion Laboratory?near Los Angeles are exercising caution immediately following Curiosity's jarring, death-defying descent to the surface on Sunday night.

They plan to spend weeks putting the nuclear-powered, six-wheeled rover and its sophisticated array of instruments through a painstaking series of "health" checks before embarking on the thrust of their science mission in earnest.

The $2.5 billion Curiosity project, formally named the Mars?Science Laboratory, is NASA's first astrobiology mission since the Viking probes of the 1970s and is touted as the first fully equipped mobile geochemistry lab ever sent to a distant world.

Equipment Checkups?

After three full days on the Red Planet, "Curiosity continues to behave flawlessly" and has "executed all planned activities" without a hitch, mission manager?Michael Watkins?said at a JPL news briefing.

The latest round of equipment checks included an instrument designed to determine mineral composition of powdered rock and soil samples; one to analyze soil and atmospheric samples for organic compounds; one to detect traces of water locked in shallow mineral deposits; and another that uses particle X-rays to identify chemical elements in rocks and soils.

The very delivery of Curiosity to the surface of Mars?already has been hailed by NASA as the greatest feat of robotic spaceflight.

The car-sized rover, which flew from Earth encased in a protective capsule, blasted into the Martian sky at hypersonic speed and landed safely seven minutes later after an elaborate, daredevil descent combining a giant parachute with a rocket-pack that lowered the rover to the Martian surface on a tether.

Since then, the rover has been sending a string of early images back to Earth, relayed by two NASA satellites orbiting Mars, providing glimpses of a terrain that scientists say appear reminiscent of the Mojave Desert in?Southern California.

One shot beamed back late Wednesday night, the first taken by Curiosity of itself, shows the rover's top deck strewn with dark pebbles apparently kicked up from the ground when the craft landed. NASA scientists said the gravel does not appear to pose any risk to instruments on the vehicle.

Two separate high-resolution "Navcam" images taken of the surface show that thrust from the sky-crane rockets during descent carved out a 1.5-foot (0.5-meter) trench in the surface, exposing what appears to be Martian bedrock underneath.

When Curiosity wakes up for its fourth day on Mars, early Friday?California?time, mission controllers plan to conduct additional instrument checks and prepare the craft for an upgrade of its main computer software for surface operations. All other activities will be suspended during that upgrade, which will begin on day 5 of the mission and last four days.
